- A biometric screening is a health assessment that measures your height, weight, body mass index, blood pressure, blood cholesterol and blood glucose.
- To be eligible to earn 2025 incentives, you must complete and submit a biometric screening by Sept. 30, 2024.
- Onsite biometric screening dates and locations will be communicated in March. Employees will also have the opportunity to visit their primary care physician or visit a freestanding Quest Diagnostic facility to complete a 2024 screening. Click here to visit the Quest Diagnostics website. Please note: the 2024 campaign is not yet visible on the website.
- Sign up for an onsite biometric screening with our vendor, Quest Diagnostics.
- If you already created an account, you may log in with your username and password. You may download a form from the Quest site for your personal physician to complete or visit a Quest Diagnostics Patient Service Center.
- If you have not created an account, please use registration key: COP2024. When registering, your 8-digit employee number (including any leading zeros) will be the unique ID you will use for the system to identify who you are.

Join the U.S. Health Improvement Incentive Program
ConocoPhillips is committed to supporting healthy and productive employees. More than 80 percent of employees participate in the program annually to focus on their health. If you are enrolled in the ConocoPhillips Medical Plan, you can earn up to $1,050 in incentives. Details related to the 2024 program will be communicated in March.
- If your 2024 biometric screening results indicate your BMI is less than 30, you will receive the $250 healthy weight incentive. No further action is needed after that.
- If your 2024 biometric screening results indicate your BMI is 30 or more, you must complete a qualifying activity by Sept. 30, 2024, to earn the $250 healthy weight incentive.
Qualifying Activities
- Q1: Healthy Wage – Complete the program and achieve a 5% weight reduction.
- The challenge is an external, employee-paid program, and the prizes are funded by participant entry fees.
- Registration is currently open. Challenge starts on Jan. 18.

- Q1, Q2 & Q3: The Nutrition Roadmap Series – Computer-based program focused on providing you with tools and tips to help you with weight management and guide you toward healthy food choices. Download the helpful resource guide as a reference while reviewing the training module.
- For best results viewing the online program via Workday, we recommend using Microsoft Edge or Google Chrome. You can still view the program through Internet Explorer.
- Q2 & Q3: Wondr Health – Complete eight of 25 online sessions. Participation is limited to new members only. Priority will be given to those who have a BMI of 25 or over.
- Sign up between March 18-31, 2024. The program begins April 15, 2024.
- Q2: SPIRIT of Wellness – Earn 20,000 points during the four-week challenge. Registration will open April 18, 2024, and the challenge will run April 29-May 24, 2024.
- Show a 5% reduction in your weight before Sept. 30, 2024, from either your 2023 or 2024 biometric screening.
- Your prior years' results will be visible in your account with Quest Diagnostics. You may show a 5% reduction by:
- Submitting 2023 and 2024 results from your biometric screening; or
- Submitting your 2023 results along with a completed attestation form; or
- Submitting your 2024 results along with a completed attestation form.
- You MUST submit before and after documented results to U.S. Health Incentive email inbox.
- Your prior years' results will be visible in your account with Quest Diagnostics. You may show a 5% reduction by:
- If your 2024 biometric screening results indicate your blood pressure is less than 140/90, you will earn the $250 blood pressure incentive. No further action is needed after that.
- If your 2024 results indicate your blood pressure is 140/90 or more, employees must review the blood pressure education material and complete the attestation by Sept. 30, 2024, to earn the incentive.
Note: Both your systolic and diastolic numbers must meet the criteria of less than 140/90. If one of your blood pressure numbers is out of range, then you must review the blood pressure education material and complete the attestation.
- Non-HDL cholesterol is the difference between the total cholesterol and HDL cholesterol. The target level for non-HDL cholesterol is less than 130 milligrams per deciliter.
- If your 2024 biometric screening results indicate your non-HDL cholesterol is less than 130mg/dl, you will earn the $250 cholesterol incentive. No further action is needed after that.
- If your 2024 results indicate your non-HDL cholesterol is 130mg/dl or more, employees must review the cholesterol material and complete the attestation by Sept. 30, 2024, to earn the incentive.
- Employees will attest to one of the two choices in Workday by Sept. 30, 2024:
- Review material from at least two focus areas on the Healthy Minds Platform OR
- Participate in at least one self-guided program through eMLife. Access your Concern Health account, select "Get Services," and then select "Self-Help" to locate the eMLife portal. To create an account or log in, please use company code ConocoPhillips.
- Once you have taken action and completed one of the options above, click here to access the course attestation in Workday. Completing the activity and the attestation by Sept. 30, 2024, will earn you the $150 mental wellbeing incentive.
Note: ConocoPhillips does not track your activity or store any data related to your activity on the Healthy Minds Platform or eMLife. The only item that is saved for incentive purposes is the completion of your self-attestation in Workday.
- Attest during the 2025 U.S. Annual Benefits Enrollment (fall 2024), that you meet the definition of tobacco free to earn the $150 tobacco free incentive.
- Tobacco free definition: using tobacco products, such as smokeless tobacco, cigars, cigarettes, electronic cigarettes or other products that contain nicotine, one time or less a month, for the last six months or complete a tobacco cessation program.
- If you do not meet the definition of tobacco free, you may complete a tobacco cessation program to be considered tobacco free. Call Health Advocacy Solutions of Blue Cross and Blue Shield of Texas at 877-806-9380 or visit wellontarget.com to register for a company-sponsored tobacco cessation program.
