Join the U.S. Health Improvement Incentive Program (Program)

ConocoPhillips is committed to supporting healthy and productive employees. More than 80 percent of employees participate in the program annually to focus on their health. If you are enrolled in the ConocoPhillips Medical Plan, you can earn up to $1,050 in incentives.

  • A biometric screening is a health assessment that measures your height, weight, body mass index, blood pressure, blood cholesterol and blood glucose. 
  • To be eligible to earn 2027 incentives, you must complete and submit a biometric screening by Aug. 31, 2026.
  • Onsite biometric screening dates and locations will be communicated in March. Employees may also visit their primary care physician or visit a freestanding Quest Diagnostic facility to complete a 2026 screening. Click here to visit the Quest Diagnostics website.
  • Sign up for an onsite biometric screening with our vendor, Quest Diagnostics. 
    • If you already created an account, you may log in with your username and password. You may download a form from the Quest site for your personal physician to complete or visit a Quest Diagnostics Patient Service Center.
    • If you have not created an account, please use registration key: COP2026. When registering, your 8-digit employee number (including any leading zeros) will be the unique ID you will use for the system to identify who you are.

  • Earn your Physical Well-being Incentive of $450 by completing an onsite screening or by submitting a completed biometric screening form by Aug. 31, 2026.

  • Participate in at least one on-demand webinar offered by our Employee Assistance Plan (EAP) vendor Concern by Aug. 31, 2026. To create an account or log in, please use company code ConocoPhillips.
  • Once you have taken action and completed the webinar participation requirements, complete an attestation in My Benefits. Completing the activity and the attestation by Aug. 31, 2026, will earn you the $300 Mental Well-being Incentive.

Note: ConocoPhillips does not track your activity or store any data related to your activity on the Concern Platform. The only item that is saved for incentive purposes is the completion of your self-attestation in My Benefits.

  • Complete one of the actions below and submit an attestation in My Benefits by Aug. 31, 2026:
    • Log in to Fidelity Net Benefits and register and attend one of the "live with Q&A" virtual webinars for ConocoPhillips employees.
    • Attend an in-person meeting or workshop with Fidelity.
    • Log in to your Fidelity Account, select "Plan & Learn" and then "Learn." Scroll down and select "Join a workshop" to browse the live or on-demand workshops.
  • Once you have taken action and completed one of the options above, complete an attestation in My Benefits. Completing the activity and the attestation by Aug. 31, 2026, will earn you the $300 Financial Well-being Incentive.
