Pre-65 HDHP Base and HDHP Plan options
The HDHP Base and HDHP medical options cover a wide range of medically necessary services and supplies, including medical, preventive care, mental health/substance abuse and prescription drug coverage.

Pre-65 and Medicare Eligible

If you or a dependent are under age 65 and Medicare eligible, you have access to two custom UnitedHealthcare® Group Medicare Advantage (PPO) options, ConocoPhillips Core and ConocoPhillips Plus, which include both medical and prescription drug coverage. These custom United Healthcare® Group Medicare Advantage (PPO) options combine all the benefits of Medicare Part A (hospital coverage), Medicare Part B (doctor and outpatient care) and Medicare PDP (prescription drug plans), as well as offer extra programs and features. You must notify the Benefits Center and provide your Medicare information within 30 calendar days if you become eligible for Medicare coverage at 800-622-5501.
