Family Changes

A change in status allows you to make changes to your benefits coverage if reported in a timely manner.

Marriage

If you get married or enter in to a domestic partnership, you may enroll, change or cancel your medical, dental and/or vision coverage if you notify the Benefits Center within 30 calendar days of the event. Otherwise, you will not be able to make a change until the next Annual Benefits Enrollment period or a future qualified change-in-status event.

Call the Benefits Center at 800-622-5501 between 8 a.m. and 6 p.m. Central time, Monday through Friday or access YBR to make the request online.

You will be required to submit documentation to verify that your dependents are eligible. Failure to provide verifying proof of dependent eligibility will delay the dependent’s coverage under the Plan and can result in the dependent’s loss of coverage.

Divorce

If you divorce or dissolve your domestic partnership, you must contact the Benefits Center within 30 days to provide notification of the qualified change-in-status. Your dependents that are no longer eligible for coverage will receive notification of the option to continue coverage through COBRA.

Birth or Adoption

Growing your family is an important and exciting time in your life and we want to help you prepare. Learn about our paid and unpaid leave policies and onsite amenities in the Becoming a Parent document below.   

To ensure your child is covered for future doctor visits, remember you have 90 calendar days to add a new eligible dependent due to birth, adoption or placement for adoption.  Please note, while you have up to 90 days to enroll newborn or adopted dependents, you are encouraged to do so as soon as possible. Claims submitted for newborn or adopted dependents not enrolled in the plan will not be paid until the dependent is enrolled. Coverage will be applied retroactively to the birth or adoption date if enrolled within the 90-day enrollment period.

Terminal Illness or Death

If you or a family member is diagnosed with a terminal illness or passes away, there are resources available to assist during this difficult time.

Accelerated Benefits Option (“ABO”) for your life insurance

If you are diagnosed as having a terminal illness, with a life expectancy of twenty-four months or less, you may be eligible to receive a portion of your life insurance. This option can provide financial assistance and flexibility. Contact the Benefits Center for additional information.

Support in the event of Employee, Spouse or Dependent Death

You may wish to take advantage of a voluntary, company-paid, strictly confidential, personalized financial counseling program administered by the Ayco Company, L.P. Contact the Benefits Center for additional information. Once you express interest, an administrator from the Survivor Support Program at the Ayco Company, L.P. will contact you by telephone.

If your family member dies you should notify the Benefits Center at 800-622-5501 between 8 a.m. and 6 p.m. Central time, Monday through Friday.
