- Register for online access to your plan at retirementplans.vanguard.com. Select "Register now," fill in your Social Security number, plan number (CPSP plan # 092538, SSP plan # 093117), birth date, and ZIP code. Create a user name and password you’ll enter each time you log on to www.vanguard.com.

For pre-65 non-Medicare retirees, medical claim forms for Aetna enrollees can be located under the "Find a Form" shortcut on the Aetna website at http://www.aetna.com.
For retirees with Medicare, no claim form is required. Submit the bill with the retiree's name, retiree's Social Security number or Aetna ID number, and patient name. Check your medical ID card for the address. Use the Retiree Medical: After-Tax Benefits Change in Status (2.01MB .PDF) - Form 15424-R to enroll in retiree medical benefits.
