- Enrollment is held in the fall each year. Late enrollments are not accepted after the annual enrollment period has closed.
If you are a new hire, you must complete the forms in your packet and submit them to HR Connections within 30 days of date of hire.

- Effective January 1, 2008, Before-Tax Change in Status Effective Dates: With the exception of birth or adoption of a child, and an employee who is "newly eligible" to participate in a plan, all Before-Tax change in status forms must be received within 30 days prior to the change in status event in order for coverage to be effective the date of the event. Change in Status forms received after the change in status event will be effective the first of the month following receipt of the form.

How the Aetna DMO Insurance Plan Works
Step #1 - Select a Primary Care Dentist (PCD)
Your PCD is the dentist you choose to manage your overall dental care.
- To select a PCD, go to your custom ConocoPhillips DocFind® or call member services at 1-877-238-6200 to request a paper directory..
- Each covered family member may select his or her own PCD.
Step #2 - Visit Your PCD
- Show your member ID card when you visit your PCD for covered services.
- You will pay a copay (a flat dollar amount or a percentage of covered expenses) at the time of service.
- There are no deductibles, no dollar annual maximums and no claim forms to submit.
- Your participating dentist will submit your claims for you.
- If you need specialty care, your PCD will provide a referral. (No referral is needed for orthodontic coverage.)
